Want to do your own NFA Form 4 or 1 Paperwork? Below is a quick checklist to use if you’re doing your own NFA paperwork to either transfer or make an NFA item. You can also use this checklist as a guide, even if your dealer prepared your paperwork, and you want to double check to make sure everything required is included
ATF FORM 5320.4 Note: You will have 3 pages for each form, and you will need to print double-sided.
Front page: Each copy will need to be filled out and signed by your dealer (Form 4) or maker (Form 1) close to the bottom of the forms above where the ATF examiner signs.
Second page: You will need to properly answer each question on the questionnaire and sign and date at the bottom. Note: If you’re buying a suppressor, you will not need to give the reasons you’re wanting to acquire an NFA item. For everything else, we advise you to write down “For all lawful purposes”.
Third page: You will need to mark “0” on number of trustees at the top and skip the lines below where you would normally add the names of trustees if you were using a trust. You will then need to check the box for your preferred payment method. If paying with credit card, you will need to add your credit card’s info. Note: The CLEO’s copy will be blanked out in the payment section.
You will then mail both ATF copies along with both fingerprint cards and $200 check (if paying by check) to the address at the top of the forms. You will mail the CLEO’s copy to your local Sheriff or Chief of Police.

Third page: You will need to mark and list each trustee or co-trustee that is on your trust. Beneficiaries do not have to be listed; only the owner of the trust and any co-trustees. You will then need to mark which payment source you will be using. If paying by credit card, you will need to annotate your card’s info. Note: The CLEO’s section will be blanked out in the payment section.
Second page: On this page, you will answer the questionnaire and attach one photo. Note: You will not have to do the questionnaire and photo on the Form 4 copy page 2 since you are doing it on this questionnaire form.
You will then mail both ATF copies along with both fingerprint cards, each trustee’s questionnaire, and a $200 check (if paying by check) to the address at the top of the forms. You will then mail the CLEO copies to your local Sheriff or Chief of Police. This will include one copy of the Form 4, and one copy of each trustee’s questionnaire.
This can be a little confusing, so here is an example for a trust:
Your name is John Doe and your wife’s name is Jane Doe. You have a Trust named “Doe’s Trust”. Here is what you will need to mail in:
